Client Information Release Form
Huckleberry Youth Programs takes client confidentiality very seriously. We comply with HIPAA standards and only release information as required by law, with client consent, or in some extreme cases when the information is critical for someone’s imminent safety and the law allows for disclosure. When information is released, we will share only the minimum amount of information required to meet the request.
