TITLE:  Clinical Case Manager

DIVISION:   Huckleberry Youth Health Center

SALARY:    $45,856 – $50,223/year DOE

STATUS/SCHEDULE:   Exempt/Union/FT (Occasional evenings and weekends)

SUPERVISOR:     Client Services Coordinator

POSITION SUMMARY:

The Clinical Case Manager is a member of the Client Services Team at the Huckleberry Youth Health Center. This position provides engagement, assessment and intervention services including case management and supportive services for youth aged 12-24. We are seeking someone who is passionate about working with youth and who is also skilled in providing strengths-based, culturally relevant, and youth-centered services within an interdisciplinary, collaborative setting.

ESSENTIAL FUNCTIONS:

  1. Provide engagement and assessment for youth who access the Huckleberry Youth Health Center and Cole Street Youth Clinic. This includes checking-in youth for their appointments, answering the phone, logging youth visits and engaging with youth in the waiting areas.
  2. Collaborate with Client Services staff to maintain a front desk and client waiting area that is safe, hospitable, and youth-centered.
  3. Provide screening for youth in order to build rapport, assess for potential needs, and share information about and/or link, if indicated, to Huckleberry and other relevant services.
  4. Provide strengths-based, youth-centered, and culturally relevant crisis intervention and de-escalation support, short- and long-term case management, and supportive counseling services.       This may include collaborating with the youth to identify specific, measurable goals related to health, mental health, education, employment, housing, relationships, substance use, and safety; providing information, referrals, and assisting with linkages to resources and services; and engaging in client-centered advocacy within and across systems (e.g. medical, housing) including coordination of resources and support.
  5. Develop, monitor, and implement individualized service plans and interventions.
  6. If education and experience allows, use psychosocial methods to empower youth to realize their own potential and adapt within their intrapersonal and interpersonal relationships and environments.
  7. Build and maintain relationships with key internal and external service providers to assist in developing a robust referral network, outreach about case management services and ensure that clients’ needs are met as seamlessly as possible.
  8. If indicated, create and facilitate group programming at HYHC.
  9. Perform accurate and timely record-keeping and documentation utilizing our web-based data system.
  10. Participate in staff development activities including but not limited to individual and group supervision, consultation and client review, and staff meetings and training to promote high quality services and ongoing program development.
  11. Step in as Client Services lead staff when Coordinator is out of office.
  12. Uphold the ethical and legal standards of the agency and field including but not limited to confidentiality, self-determination, and mandated reporting.
  13. Keep abreast of current research, curriculum, and best practices in adolescent and behavioral health and serve as a resource in the community and agency for working with youth.
  14. Other duties as assigned.

COMPETENCIES:

  1. Experience working with culturally diverse youth who may be underserved, marginalized, in high risk situations, and/or living with adverse childhood experiences (ACEs)
  2. Experience providing clinical case management services in collaboration with youth
  3. Experience working in an adolescent clinic or other integrated health setting that offers a combination of services including but not limited to sexual, reproductive, and/or mental health care
  4. Experience working within a youth development, harm reduction, trauma-informed, and/or social justice framework
  5. Must be highly organized, creative, innovative and open to new concepts
  6. Able to handle competing demands, effectively prioritize tasks, and take initiative while focusing on the needs and experiences of youth
  7. Ability to exercise sound, independent, professional judgment and decision-making within the legal and ethical standards of the field while also maintaining a focus on the needs and experiences of youth

 

SUPERVISORY RESPONSIBILITIES:

This position has no supervisory responsibilities.

 

WORK ENVIRONMENT:

This position operates out of the Huckleberry Youth Health Center. This role may include driving agency vehicles to various sites across San Francisco on occasion. The position also includes the use of standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Huckleberry Youth Health Center is located in San Francisco and is accessible by bus.

 

PHYSICAL DEMANDS:

This role requires the ability to lift files, open filing cabinets and bend, sit or stand as necessary and climb stairs.

 

REQUIRED EDUCATION AND EXPERIENCE:

  1. Master’s degree in Social Work, Counseling, Psychology, or similar field
  2. Computer literate (Internet, Google Suites, Word, Excel)

PREFERRED EDUCATION AND EXPERIENCE:

  1. Fluency in Spanish or Cantonese
  2. Knowledge of San Francisco and Bay Area community-based agencies and resources for youth
  3. Experience working with youth who are homeless and/or marginally housed, system-engaged youth, youth of color, LGBTQQ youth, young women, and/or transitional aged youth

 

ADDITIONAL ELIGIBILITY QUALIFICATIONS:

  1. TB clearance
  2. Valid CA driver’s license & clear DMV record

 

SECURITY CLEARANCE:

  1. Must pass DOJ/FBI background check
