TITLE:                          Academic Case Manager

SALARY:                      $14.62-$16.85 per hour + .50¢ per hour for Bilingual Capacity
STATUS/SCHEDULE:   Non-Exempt
SUPERVISOR:              Director of Wellness Academy, Marin 

AGENCY DESCRIPTION:
Huckleberry Youth Programs (HYP) is a critical provider of essential safety net services for high-needs youth and their families in San Francisco and Marin County. For over 40 years, HYP has developed and maintained prevention programs that promote the health and leadership of youth, and, as a result, divert youth from violence, crime and gang involvement, as well as other harmful behaviors, including running away, drug/alcohol abuse, risky sexual activities, and involvement in the juvenile justice system. The agency employs 50 staff members, and operates on a $4 million annual budget.

RESPONSIBILITIES:
Under the supervision of the Huckleberry Wellness Academy Director, the Academic Case Manager is responsible for working with Huckleberry Wellness Academy participants to achieve academic success and enter post-secondary educational programs related to careers in the health sector.  The Academic Case Manager will provide comprehensive group and individual support to youth.  As part of the Wellness Academy team, they will work to: a) provide individual academic case management, b) facilitate Wellness Academy group meetings, c) organize parent trainings, d) conduct community outreach, and e) participate in data collection activities

DUTIES:

  1. Work with the Wellness Academy team to develop, review and revise curriculum and training modules as needed introducing youth to health careers, college readiness, job skills and peer health education. Co-lead weekly afterschool groups (sometimes 2 -3 per week).
  2. Work individually with Wellness Academy participants on issues related to grades, graduation and college entrance requirements and other academic concerns; including tutoring or teaching organizational skills as needed.
  3. Work closely with High School counselors and teachers to monitor participants’ academic performance as well as completion of a-g requirements
  4. Coordinate referrals and linkages for participants as needed
  5. Assist in the planning and coordination of parent support/parent outreach activities
  6. Participate in weekly supervision and other program and agency meetings
  7. Collect data on all program activities
  8. Other duties as assigned.

QUALIFICATIONS:       

  1. Bachelor’s degree in Education, human/social services or similar degree preferred
  2. Experience working with and relating to youth, youth of color, and their families and communities. Cultural sensitivity and competency required.
  3. Experience working in direct service with youth including training and facilitating group discussions on topics related to academic achievement, employment, job readiness, and health or/or social justice (preferably with high school youth).
  4. Experience in working in partnership with youth
  5. Demonstrated ability to be creative, innovative, and motivated (and encouraging others to do the same)
  6. Commitment to the goal of reducing the educational achievement gap and reducing health disparities.
  7. Criminal Justice clearance and valid CA driver’s license, clean DMV record and 2 yrs previous driving experience
  8. Bilingual English/Spanish preferred
  9. Computer literate (Internet, Word)
  10. Must be available for occasional early evening and some Saturday shifts
  11. Must be available for occasional trainings and be able to attend meetings in San Francisco.
  12. Must be able to lift & carry objects up to 10 pounds unassisted on an occasional basis.
